Coming Up Short

The physician shortage is a concern for all medical professionals, proving that a lack of numbers means a lack of strength. Here’s a look at industry stats, consequences and needs.

Between dealing with the demands of radiologists and referring physicians, imaging technologists can be excused for thinking that the United States has too many physicians rather than too few. But the truth can’t be ignored any longer: This country is suffering from a deepening deficit of doctors.

For years, many analysts and academics actually projected that the United States would have far too many physicians unless training was reduced. A few still feel this way. But for most observers, the jury is in: Unless we train more physicians, shortages will undermine timely access to health care.

The voices warning of this shortage are many and varied. In the Feb. 20, 2006, edition of American Medical News, J. Edward Hill, MD, then president of the American Medical Association, vowed that the AMA wouldn’t ignore the problem, which he said was as serious as a pandemic.1 In 2006, the Association of American Medical Colleges committed to raising medical school enrollment 30 percent by 2015. Ninety-six percent of hospital CEOs surveyed recently by AMN Healthcare Inc. of San Diego (the parent company of Med Travelers of Irving, Texas) said that the United States should begin training more physicians immediately. In February, Sen. Bill Nelson (D-Fla.) and Harry Reid (D-Nev.) introduced the Resident Physician Shortage Reduction Act, which would increase physician training slots by over 1,000.

Many people wonder how we could possibly be running short of physicians. The problem isn’t finding enough young people interested in medicine; rather, it’s finding the resources and will to train them. Physician training is expensive, and on the residency level, it’s largely funded through Medicare. To train more physicians, we’d need to spend more on Medicare—an area of the federal budget that many politicians want to cut.

That’s one reason why the number of physicians trained in the United States has remained at roughly 16,000 per year for the last 25 years. In fact, although more women are becoming doctors, the net supply of physicians is actually decreasing because female physicians on average work fewer hours than their male counterparts.2

The U.S. Census Bureau projects 45 to 50 million more Americans between 2000 and 2020. The population is also aging, and the Department of Health and Human Services indicates that patients 65 and older visit physicians three times more than younger adults. And image-conscious young adults and baby boomers are demanding procedures ranging from the realm of Botox to bariatrics.

The shortage is felt most acutely in emergency rooms, where not enough specialists are willing or able to cover the ER. When hospitals can’t provide specialty care, patients are transferred to another facility—a time-consuming process that can jeopardize lives, particularly for head trauma cases. The issue of patient access to more routine care also is being played out in the ER, where insured patients are presenting because they refuse to wait weeks to see their doctor for a headache or backache.

To some extent, teleradiology has eased the strain of the radiologist shortage. Nevertheless, many facilities—particularly in medium to smaller communities—still struggle with radiology staffing. Along with a tight radiologist market, technologists can expect the number of patients seen by primary care physicians and specialists to grow further. As patient backlogs develop, pressure will mount to enhance study turnaround times. Maintaining quality and patient rapport in a system stressed by patient bottlenecks will be challenging. Patients and physicians are likely to be even more irritable and distracted—and when physicians, the leaders of the medical team, are overworked or absent, everyone feels the repercussions.

Even if Congress increased physician training immediately—a big “if,” at this point—new physicians wouldn’t be in the system for years. Unfortunately, this shortage will be a hard fact of life that patients and health care professionals will face for the foreseeable future.
